I've seen pictures and vids of a case called SYLWIA. I'd be interested in getting one as it places the DVD ontop of the case. The pictures I've seen are marked bonzo.pl but I haven't came up with anything searching google. Don't know if it's someone who made it custom or what but I'd sure like to get my hands on it!

Maybe this is what you are after:

The picture above looks like it's custom made.. I haven't seen anything produced like that. The Xdrive looks nice, but i don't know if they are still available.

Yea I think it's custom made too, I might have to get an external DVD drive enclosure and mold in into the top of my 360.
